Okay, so. In Swan Song, Sam merely repressed Lucifer when he fell into the pit. There was nothing anywhere that said that Sam and Lucifer separated after they went to Hell.
I believe that Lucifer was actually trapped within Sam’s body and his soul when they went into the pit, and when Sam returned soulless, Lucifer was still trapped in the pit, and in Sam’s soul.
